What do I have to do to develop a Computer-Guided Surgery case using the 3D-PILOT method?

The first thing to do is ask Biotec how to contact the nearest BTK Template Development Centre.

You will be guided through the initial phases of preparing patient models and prosthesis planning.

 

What are the main phases foreseen by the 3D-PILOT method?

• Selection of the surgical case

• Contact with the Template Development Centre: preliminary prosthesis design

• Patient imprint and mastication mould (assembly of teeth where necessary)

- Sending of gypsum models to the Template Development Centre

• Receipt of the radiological template with 3-D marker

• Patient CT scan

• Sending of the DICOM file and receipt of the software license

• Implant planning

• Sending of the implant project to Biotec

• Receiving of:

- Realpatient CAD-CAM anatomic model

- surgical template

- provisional (where requested)

• Surgical operation.

 

Which gypsum models should I prepare?

The required models vary according to the surgical case and the preliminary prosthetic design performed with the support of the Template Development Centre. This is why direct contact with the BTK authorised Template Development Centre is vital.

 

I was not aware of the radiological template and I already have the patient’s CT scan. Can I still use guided surgery for this case?

It depends on the quality of the image: if you can distinguish the occlusal surfaces of the teeth and there are not too many metal artefacts, it is possible to plan the case. In the presence of total edentulism, it is necessary to force open the flap.

Without a radiological template therefore, it will not be possible to perform flapless surgery.

 

What is the red brick for?

The 3-D marker is used to couple the scan of the gypsum model and the patient’s dental prosthesis with the patient’s own CT scan. This makes it possible to perform prosthetically-guided implant planning.

Secondly, it will be possible to generate the patient’s anatomic model using CAD-CAM technology.

 

Who scans the models?

The scans of the model and radiological template are performed by the Template Development Centre.

 

How can I go about using the software as a diagnosis instrument only?

Simply contact Biotec as soon as you receive the radiology CD.

On connecting to the network, a Biotec technician will recover all the information needed to convert it to a software license.

 

Is there a specific protocol to follow for the CT scan?

When doing a CT scan, it is essential that the radiological template fits the mouth perfectly and is firmly in place.

The CT scan is performed with mouth-occlusion in order to stabilise the radiological template.

The 3-D marker must be completely incorporated in the scan.

 

Which files must I request from radiology?

To generate the software license you must request the non-compressed axial images in DICOM format from radiology.

 

Is a double CT scan required?

No. Just one CT scan is required. The patient must be wearing the radiological template and, in the case of total edentulism, the silicon occlusal index.

 

What is the CAD-CAM milled model for?

The Realpatient CAD-CAM model is used by the Template Development Centre to build the surgical template.

Furthermore, if requested, this model can also be used to develop a provisional dental prosthesis before the surgical operation, so that the post-surgical imprint is not required.

 

Can my technician perform the radiological template?

For the first few guided surgical cases, we recommend you contact an authorised Template Development Centre only.

Our technicians are however at your disposal to transfer the information required to develop the radiological template to your technician.
